At Aware CDI, we offer comprehensive NDIS Support Coordination services designed to help you get the most out of your plan. Our person-centred approach means we take the time to understand your unique goals and work alongside you to achieve them. Here’s how we support you:
1. Understanding and Using Your NDIS Plan
We help you make sense of your NDIS plan by explaining your funding and supports in a clear, practical way. Whether it’s setting meaningful goals or identifying how to use your funding most effectively, we guide you through each step to ensure you feel confident and in control.
2. Connecting You with the Right Providers
Finding the right service providers can be overwhelming. We take the time to understand your preferences and needs, then connect you with providers that are the right fit. We also help negotiate and establish service agreements that are clear, fair, and aligned with your goals. Our team keeps an eye on quality and advocates for your rights to ensure you’re receiving the best possible support.
3. Coordinating and Scheduling Supports
We help manage the day-to-day logistics of your support services. From scheduling appointments to organising transport and managing communication with multiple providers, we act as a central point of contact to keep everything running smoothly. We also help track and organise important documents, reports, and updates.
4. Building Skills and Independence
We work with you to build everyday skills that promote independence, confidence, and self-advocacy. This includes helping you engage with your community, join social activities, and develop the tools needed to manage your supports and make informed choices about your life.
5. Preparing for Plan Reviews
When it’s time for your NDIS plan review, we help gather all the necessary documentation, reports, and evidence. We work with you to reassess your goals and ensure they reflect your current needs. We also advocate on your behalf to support a fair and appropriate funding outcome.
6. Removing Barriers to Access
For participants with more complex needs, we offer a higher level of coordination and support. This includes crisis resolution, transition planning (such as moving to supported accommodation or leaving hospital), and managing multiple supports across services. Our team is experienced in working with individuals and families facing challenging circumstances and ensures no one falls through the cracks.

Tailored Support for Every Participant
WHO WE SUPPORT
At Aware CDI, we provide specialised support tailored to the unique needs of each participant. Our team brings extensive experience and local insight to deliver meaningful, culturally competent, and age-appropriate services.
We support children and young people through key life stages, including early intervention, school transitions, post school pathways, and independent living goals. Our aim is to build strong foundations that promote development, inclusion, and future independence.
Our team works alongside adults living with physical, intellectual, and psychosocial disabilities to coordinate the right mix of supports. Whether it's daily living assistance, community participation, or employment pathways, we tailor our approach to each individual’s goals and lifestyle.
We proudly offer culturally sensitive and inclusive support to Deaf and hard-of-hearing participants. Our team includes staff experienced in working with Auslan users, ensuring communication is accessible and respectful at every step.
With a strong presence in the Torquay, Geelong, Surf Coast, and Bellarine regions, we bring valuable local knowledge to connect you with trusted providers, community resources, and inclusive opportunities that enhance everyday life.
At Aware CDI, we are committed to providing support that is culturally safe, inclusive, and respectful. We acknowledge the deep history, strength, and wisdom of Aboriginal and Torres Strait Islander peoples, and we recognise the importance of community, Country, and connection to identity and wellbeing.
Aware is committed to:
- Taking the time to listen to your story and cultural needs
- Respecting your choice of services and the people you feel safe working with
- Partnering with Aboriginal-led and community-trusted services
- Advocating for your goals in ways that honour your values, identity, and community
